All athletes must have a physical and participation form on file before practice begins.  The forms can be printed using the links below or picked up in the office of Flowers Hall.  The participation form must be notarized; notary service is available at the font desk of Flowers Hall.  The physical form must be filled out by a physician and turned in to the front desk of Flowers Hall or to Coach East.  These forms expire after 1 calendar year.
